Frequently Asked Questions

How do I request a copy of my medical records?

Complete an Authorization for Release of Information Form.  You can obtain a copy of the form by clicking the following link: www.milfordregionalphysicians.org/downloads.  You may also request a copy of the form by calling 508-473-1480 ext. 294; please leave a message stating where the form can be emailed, mailed or faxed to your attention.  Records requests can also be made via Patient Gateway by clicking the following link: https://mychart.partners.org/mychart-prd.

How can I submit my request?

Send the completed Authorization for Release of Information Form via mail to the address listed above, via email to MRPG-medicalrecords@milreg.org, or via fax to 508-478-0694.

Is there a cost to obtain medical records?

The cost will vary depending on the request. Once the request is reviewed, an invoice will be mailed to you.  We do offer patients a comprehensive "Transfer of Care" option at no charge when transferring care to a new provider.

How do I submit payment?

Payments can be made by check or by credit card.  Credit card information can be accepted via phone.

How do I get just my immunization information?

Contact your Physician's office if you only need immunization history.  They will be able to provide you this information.

What is the processing time for medical records?

Processing times vary based on the request. Routine requests are processed within 3-5 business days after payment is received.

How do I request records for a deceased patient?

Legal documentation must accompany the authorization release form. Various documents are acceptable if they clearly designate your authority as the "authorized personal representative" to request medical records. If you do not have such documentation, contact your local Family and Probate Court to inquire about becoming an "authorized personal representative".
