Blackstone Valley Obstetrics & Gynecology at Whitinsville Medical Center is committed to providing quality health care to the women of the Blackstone Valley in a warm and compassionate environment. As a husband and wife team with twenty years experience in labor, delivery, urogynecology and pelvic reconstructive surgery, Dr. Samuel Zylstra and Brenda Zylstra are trained to evaluate total female health needs. They provide personal health care and are specially trained in the medical and surgical management of obstetrics and female pelvic floor disorders. They are both board certified in their respective areas.
Our services have been designed to offer you the best possible care. When you are ill, we are able to see you on the day that you call. And, if you need additional care, on-site x-ray, physical therapy and laboratory services provided by Milford Regional Medical Center mean you won't have to travel far.
Office Hours
We see patients during the following hours. Please note that while our office may be open for patient visits, our phones may be covered by our answering service before 8:00 a.m. and after 4:30 p.m.
- Monday 1:00 p.m. to 5:00 p.m.
- Tuesday 12:00 p.m. to 8:00 p.m.
- Wednesday 9:00 a.m. to 5:00 p.m.
- Thursday 1:00 p.m. to 5:00 p.m.
- Friday 9:00 a.m. to 5:00 p.m.
Our office is closed on holidays.
Scheduling an Appointment
To schedule an appointment with Blackstone Valley Ob/Gyn, please call 508-234-6260. If you need to cancel or reschedule an appointment, please give us 24 hours notice so that we may schedule that time with another patient. If we do not receive notification, a no-show charge may be billed to your account.
Prescription Refills
To renew a prescription, call our office during regular office hours. Please allow forty-eight hours for routine refills. Sometimes a prescription refill request appears simple, but may require an examination or new instructions. In these cases, we may request that you see your provider. You should have all medicine names and doses with you for appointments and phone calls.
Referrals
Your providers are sub-specialists in urogynecology (also called female pelvic medicine and surgery). Your doctor may recommend that you see other sub-specialists such as infertility, perinatology or gynecologic oncology. Most referrals require your primary care physician to evaluate and refer you.
Insurance
We are providers for most medical insurance plans. Because there are a wide variety of insurance options, please contact your insurance company directly to make sure your doctor accepts your plan.
If you are insured, please remember to bring your insurance card with you when you visit our office. We must validate your insurance at each visit. Your co-payment is due at the time of your visit. We accept cash, check, Mastercard, Visa, Discover and American Express.

Our primary hospital is Milford Regional Medical Center. We are also affiliated with Harrington Memorial Hospital.
